to submit a takedown request under the dmca, send a notice including:

• your contact information (name, address, telephone, email)
• identification of the copyrighted work claimed

• a statement of good-faith belief that use is unauthorized
• a statement under penalty of perjury that the information is accurate

counter-notice
if you believe your content was wrongly removed, provide:

• your contact information and a description of the removed material
• location of the material before removal
• a statement under penalty of perjury your use is lawful
• consent to jurisdiction of your local federal court

repeat infringer policy
we reserve the right to terminate accounts of repeat infringers in appropriate circumstances.
